If there were ANYTHING that had the power to force Sam's hand, I really quite honestly thought it would be that.
Yes, I had the same reaction. On reflection, I have concluded that Sam's powers were indeed passive, outside his control whether conscious or sub-conscious. I can't believe if he could access them even sub-consciously that he wouldn't have done so in time to save Dean. We still don't know exactly what happened with Lilith - whether she was repelled by something within Sam, or whether Sam actively repelled her, but I came to the conclusion that it happened because she turned her powers against Sam's and that this is what woke them from their dormant state.
If there is any mileage in this view, then it must be something very specific to Lilith / YED / selected others, since it didn't happen with Lilith's minion in Malleus Maleficarum, but we know that YED and Lilith are on a different pay-grade from other demons the boys have encountered so I don't think this is completely out of the question.
Right now, I'm just excited about season four. What could happen, what people could make of it. The angst potential is incredible, and I'm really hoping against hope that if/when they get Dean back, he'll be changed, a tortured soul. Dude, PTSD anyone? *bounces*
I think for me the thing with the deal is: all it said was you die and you go to hell. Now, Dean has fulfilled his part of the deal in the season ender: he died and he went to hell. Nothing said he had to STAY dead or STAY in hell.
So, anything after this, regardless if he escapes and/or comes back to life, he fullfilled his end of the deal. It's done as of now :D
