North Eastern Colorado here.

I sang that song for years in choir in both elementary and junior high. I think as a gag my high school choir director made us use that for a warm up a couple times. It's rampant up here.

Then again, I went to very musical schools, for all they were the "hick" schools.

I still teach that song at Girl Scout camp in the Poconos of PA, there is a second verse and a car version:

Kentucky fried chicken and a pizza a hut (x2)

Good lord, I don't believe I've heard that song since about 4th grade or so. I think I learned it on long bus rides on field trips and such... I'm not sure who knew it originally. But it was most definitely here in Baltimore, MD and probably still is.
