Does this make Jack officially the worst boss ever?
Three of his employees have now murdered him on three separate occasions. He must be doing something wrong...

Either Jack is the worst boss, or Ianto dosen't have a clue yet that Jack is immortal and when he comes back, Ianto is in worst trouble than ever. I was definitely surprised with this one. Even though Ianto worked at Torchwood London, didn't he know the Cybermen were lethal and Lisa could not be saved?

I think it's a combo of both. Ianto seemed to calm when later in series 1 when Owen shot Jack, so I think it was sometime after Cyberwoman that Ianto learned about Jack.

I believe love blinded Ianto where Lisa was concerned.

I would agree that Ianto blinded himself where Lisa was concerned but I wonder just how much Torchwood One really knew about the Cybermen and how people were converted? From his reaction to what Jack did to Lisa, I honestly don't think Ianto had any true idea of what that conversion process entailed; if he did, he would have known she was already dead. I wonder if anyone has ever covered conversion as a prequel to a Torchwood One story?
