Sam/Dean, set in season 5. Sam notices that Dean developed an eating disorder after hell, exacerbated by all the stress in their lives.
Dean needs some control in his life and not eating is how he's chosen to get it; to him, food doesn't taste like anything. Sam is massively concerned, guilty it took him so long to notice and determined to help Dean somehow. Cuddling is encouraged, as is supportive!Sam.
Sam, Dean - gen or slash, writer's choice - Dean needs oral surgery. Afterward, his face/jaw is puffed out like a rabid chipmunk [cue horrified onlookers!] and Sam has to mash all his food and buy him ice cream/soup etc.
Believe it or not, I have something started already that could work here. I'll post the first bit in a sec, and the whole thing once I finish it up. :)
“Dude,” Sam says expressively, wrinkling his nose.
“Geh oo eyeint am owa air,” Dean glares.
Sam blinks at him. “I have no idea what you just said.”
Dean bites him.
“FUCK!” Sam yanks his hand back, wrapping the hem of his shirt around his bleeding finger. “Dean, what the hell?!”
“I asked you nicely to get your giant hand out of my mouth,” Dean defends.
“Oh, and it’s MY fault I couldn’t understand you.”
“Hey, you were the one playing amateur dentist. Hermie.”
Sam sighs. “Whatever. There’s more of that tooth gone than there is left.” He takes a swig from his beer bottle. “Who knew leprechauns were so handy with crowbars?”
“Bite me,” Dean spits before he thinks. ”SON OF A…” He actually goes a little green. “Damn, that hurts.” He reaches for his own beer and takes several long gulps. “Seriously. How bad is it

How about some pre-series Dean getting impaled on something. The more detail and freaking out, the better. Sam and John do their things. Also, don't make John an a$$hole. Purty please?!?
Dean didn’t even feel anything the moment it happened. It felt as though it was happening to someone else and he was just an observer, looking in on an event he wasn’t really a part of. He could hear Sam and his dad yelling; calling out his name in the same panicked fashion they always did when he found himself on the receiving end of an injury

Gen or Slash, writers choice - If someone can figure out a way for Dean to have to wear a patch [omg, I can hardly type this I'm laughing too hard] for like a week? while his eye heals from something, I would love you forever....

Dean didn’t look so good. His skin was damp-so pale that his freckles stood out. The skin around his left eye was bruised, his other eye covered with a patch and bandages. He looked sick. And so pathetic lying on the bed unable to move. Doctor’s orders.
And Sam wasn’t trying to be a dick while Dean wasn’t well. He knew that Dean needed rest to recover from his eye surgery, it was just... it was too good an opportunity not to pass up. And, well, he couldn’t pretend Dean wouldn’t do the same to him
