That was gorgeous. Wonderful idea and execution, and I love how the dreamy sort of writing style emphasizes the almost trance-like state Ten finds himself in here. In terms of pacing, it takes a while to figure out quite what's going on, but in a really good and poignant way. The way we find out about the pregnancy, and feel the shock right alongside Ten? Perfect.
I especially love how you convey the Doctor's mixed feelings with the repetition of his 'shouldn't be here' thoughts, and I loved this sentence:
Is he glad (or bitter, or jealous, or disappointed) that it took them (only, a whole) five years to move on, to carve a life of their own?
The story overall... it's not heartbreaking, but not happy either. It just works. I really really enjoyed it.

It's so emotive and sad. This one is heartbreaking but... at the same time hopeful in a bittersweet way.
