Lovino is a prostitute (preferably from a brothel) and Antonio becomes a regular customer. It starts off all physical, but it eventually develops into love.

I have to say my heart literary skipped a beat when I read this, because this is EXACTLY what I've been wanting to request for the longest time, but haven't been able to now with the request freeze. You've no idea how this idea has been circling in my mind anon :'D The idea of Spain going to a brothel and ending up falling in love with Romano, the prostitute, and ending up practically bankrupt because he can't stop spending money on sessions just to see him (because we all know Spain has very poor economical judgement when it comes to Romano, no?)
