A story similar to the one you have described is Are you ready for love by Moonshadow Woman

Summary - Spoilers Warning! - Highlight to read
Justin and Brian split completely after the Rage party, years later Justin is an acclaimed writer and artist. After seeing an interview of Justin with Oprah, Brian decides to get in contanct with him again and finds out that he lives in a house just outside of Pittsburg and has adopted two HIV+ kids. They slowly get back together. One day Brian accepts the request of a social worker to take in an HIV+ baby girl who ufortunatly later dies. While visiting her grave, Brian meets and becomes friends with an orphaned HIV+ boy who evetually gets adopted by them.
