I recently had to apply for my first passport because I'll be leaving the country in about 3 weeks (it was sort of sudden). I had my name change somewhere around 10 years ago. My name change was only 4 years ago and after the first year, I stopped listing my previous name on any applications I had to do. I got my first passport in 2011 with no issues.

ETA: I do not know (and did not research) if it is really necessary to list former names on passport applications. This is just my personal experience that I didn't and it worked out.

Good luck!

I would in inquire with your passport office. If everything up to date matches (social, address, name, ect) they should be able to identify you as you.

Only if you had a previous passport will they ask about it or flag it but even then if you call the office they can have you come in for a face to face and they issue the passport the same day at the local office. Either way you should be fine.
