I bled a good bit after my surgery. Not, like, waking up in a pool of it or something, but I -did- leak. [ I had no gauze, just tape and stuffing around it to pad me. ] I called my surgeon just to let him know but did nothing else.
It's really your call. It's your body...what is it telling you? =]

I had a burst stitch on my nipple, there was only 4 or 5 of them so it looked like quite a bit of tear. It bled a few times but sorted itself out. Though it did my scars no favors and I lost a tiny bit of my nipple because of it- but that won't be a problem for you. Call him and see what he thinks. I just left mine's because it seemed to me to be not much blood but if you're not sure of that seeing a doctor face to face wouldn't hurt.

I kept my binder on for 6 wks except to shower and was told to keep my elbows close to my sides I had the perri so different than you---- but lisen to your body you know it best and congrats on your surgery.:)
