I am now thinking perhaps it is some kind of strange sign of approval or acceptance that shop people (mostly pharmacists) speak english to me almost immediately

What's stranger to me is when I am in a store, and somehow muddle through the whole process in German, then hear them turn around and speak to their coworkers in fluent English.
That happened to me a lot when I first moved here, I spoke nearly no French, and while I have never been in a situation where they turn and speak to their coworkers in Dutch, they have spoken German, which is much more familiar to me.
I wish it were just easier for me to learn languages. It has been nearly 5 years for me to feel comfortable in French, German was nearly as long, and I still feel strange in English and I may never speak Italian normally.
