His world ended.
That world was torn away from him, faded to black and white stained with red and ended with only one thing on his mind: revenge.
The problem was, though, that he hadn't ended. His dreams were filled with his mother's voice, his father's smile, the memories of clan he loved, and worst of all, his brother. Not the killer that had destroyed his world. But the brother he idolized, who flicked his forehead, was gentle with him and silently uttered 'I love you' every day. They were nightmares that he couldn't hide from, that he wasn't even sure that he wanted to because they were the only place that their faces and voices didn't fade. He dare not look at pictures, that was only for the weak. There was no way to hear them again.
Without even realizing it, though, he started to build a new world.
It hit him sharply when Sakura was bound to that tree and Naruto was down; Gaara was going to kill them. It felt like his brother slashing his parents all over again. It was like someone had ripped out his broken heart and was threatening to destroy it completely.
He thought: They are precious to me and I do not want them to die in front of me. I will protect them both with my life.
He forgot about Itachi.
It was something he thought about, later. It was different from the bridge, with Haku, because he had moved without thinking. This time he thought about what he wanted to do and then tried to act. Itachi's memory didn't matter, in that one single moment, only they did.
Later, when he thought about it, he had no idea what that meant, but he was sure he had some time to figure it out.
Until Itachi came back and sharply reminded him of his old world, his only purpose.
Another problem arose. He had a new world. He had new hope. And they could die. That terrified him: he could admit that when Naruto had him pressed against a cliff and telling him he was like a brother.
He clung to his old world, walked a dark, lonely, bloody path with a small smile. He wasn't Naruto, he never claimed to be brave and his definition of strength was power. He gave up love as his defining point when he lost everything that mattered to him in one single blow. The memory of his tears falling down his face is burned into his mind and heart as clearly as his dead parents.
He told Orochimaru that he didn't care about what he had left, the only thing that mattered was his past. He didn't kill Naruto because he wasn't Itachi and refused to become him. Orochimaru didn't have to know he thanked Sakura or that Kakashi's word had any sort of impact on him; Orochimaru already doubted Sasuke's resolve, he whispered this in Sasuke's neck as he held his arms and showed him how to properly take a life.
It was impossible to lie to himself about his new world. But his world died once without warning so he was sure it was a small matter to shatter his new one. His heart is broken beyond repair, anyway.
