Transportation Secretary, everyone knows that's the least import cabinet position, right?

Hell! What's so important about transportation planning anyway? It's pretty simple right? I mean the Department of Transportation is only in charge of little things like safety for land, air and sea transportation in the nation. How complex is that?

The video is correct, and yes, many people working on transportation safety issues are trained in these things. Furthermore, the plantings you mention can be designed in such a way as to treat stormwater runoff, too, and some municipalities are doing a lot of this. There are many things that can be done (and are being done in many areas) to enhance transportation choices, but what safety tools might work best depend on a variety of factors like average speed, traffic and pedestrian volumes, etc. Bulb-outs are only one of several choices that can enhance pedestrian safety.
