House-training problem

OK, I will attempt to make a long story short. My family and I found this cocker spaniel almost a month ago. After three weeks of searching and signs and newspaper ads, etc, no one has claimed her so we've decided to keep her. Now I have had dogs before (mostly of the much larger variety) and never had many problems training them. But our new baby, who we've named Sasha, refuses to be house-trained.

I'm ocnfused because she seemed to be house=trained when she first got here. She would scratch at the front door and whine when she wanted to go out. No problem. Then one time, our oldest daughter was home alone (she's almost 14) and she missed te dog's signs for wanting to go out. So Sasha, likely desperate, went to a corner in our dining room and pooped. Now, ever since then, she's been pooping and peeing less and less outside where she's supposed to and more and more inside where sh's not. She's peed on the kids' beds, keeps going back to the spot in the dining room (even though we cleaned it with febreeze AND Nature's Miracle). When outside, she romps and plays and walks nicely... but shows little desire to do her business. Only time she goes potty outside is first thing in the morning. She won't pee or poop in the bedroom where she sleeps, but everywhere else seems to be fair game. We praise her like crazy when she poops outside and we shoo her away when she poops inside, but she just doesn't seem to get it.

Any ideas on how to re-house-train an adult cocker spaniel? It's not a health problem, because she saw the vet last week and was pronounced to be in perfect health. And aside from this one problem, she's a wonderful dog. The hubby and I are tired of having to do laundry or carpet cleaning every day because of this dog. He's starting to want to get rid of her, but we're all already attached.

And before you suggest crates, we already tried that. She is TERRIFIED of being contained, goes cage crazy to the point where we were afraid she would hurt herself. She doesn't like being confined in the bathroom alone either (which we've been doing with her when we all leave the house, just to head her off since if she pees or poops in there, at least it's easily cleaned). She has no problem being confined if someone is with her (ie, being in the bathroom with me while I'm in the shower).

So, does anyone have any advice on how to remedy this problem? She's been getting walked four rimes a day and for at least three of them she doesn't do anything.
