Ah, spyglass, I have a (physically) successful shoulder surgery story. The secret is a really good surgeon, IMO. I'd be happy to give you the name of mine, but he's in Oakland. They talked about shots for me, but once the MRI showed a full-thickness tear, they moved to a surgery recommendation pretty quick. I, too, have heard that one needs to minimize the shots.
I had lots of drugs after surgery, but felt almost immediate improvement over how it had been (chronic and painful). I had a really slow healing cycle (expected) but a year later and it's hugely improved.
For a rotator cuff tear, the surgeon told me that really time and rest was most important for healing. If you have a different injury, YMMV. Send me email if you have any questions you think I can answer.
