Sometime during Sam's time away at college Dean and John start up a relationship. Dean becomes obsessed with the idea of possibly knocking up John, and eventually succeeds. Up to writer whether it's because John's suppressors fail, Dean sabotage's his birth control, John gets sloppy thinking he's no longer fertile anyway, whatever.
Cue Dean spoiling John rotten through pregnancy and being all protective alpha. John really flourishes under Dean's attention and in typical omega fashion gains the pregnancy weight and then some.
Up to writer whether John is big because multiples or simply the craving indulgence and Dean's spoiling.
I just really want some pregnant!John and would be ecstatic if it could be daddycest (I will accept John/Mary as well if Daddycest is too big a squick)
Dean would be lying if he had said he had never thought about it before. Being an Alpha with an Omega parent it would have been strange if he hadn’t fantasized about it at some point. Omegas were meant to be bred and Alphas were there to do the breeding

Dean soon found out that it was an old wive’s tale that an Alpha could smell pregnancy on an Omega. Dean’s verification came a few weeks later when his dad, who Dean couldn’t remember being sick a day in his life, was subject to vomiting spells morning, noon and night. Morning sickness, it turned out, was just a phrase instead of a guideline. John was sick more than Dean thought a person could be and he became immediately protective and possessive. They couldn’t get much work done with as much time as John was spending with his head hanging over the toilet so they switched to research mode as Dean did his best to pamper his father. Once he found foods that he was able to keep down Dean fed them to him as often as he could. They didn’t talk much, or at all, about the baby at first but it was clear that John had no intentions of getting rid of it. Whether he was looking for a replacement Sam or giving into the loud ticking of his own biological clock Dean wasn’t sure. All Dean knew was that it took a good amount of restraint to stop

There were days, even weeks on end, where John didn’t necessarily forget about the mission but started thinking that maybe it wasn’t the most important thing in the world. Even when John entered the third trimester already bigger than a house there weren’t many complaints outside of the average pregnancy stuff. Dean massaged his father’s swollen ankles, helped him to the bathroom and tried his best to calm the growing kicking children growing inside of him
