Dean thought getting pregnant was hard but a few rounds of fertility treatments and birth of triplets later he finds that getting any of the weight off to be much harder. He's uncomfortable in his post-pregnancy body and his appetite hasn't eased off much so at his first weigh in he's actually put on weight and the number continues to go up and up which actually makes him like his shape more.
Dean's weight problems started after his mother died and he had a life time of his weight going up and down the scale due to John putting him on diets, giving him exercise regimes and maybe even sending him to a camp once or twice. At his smallest he's still been undeniably chubby. Before John dies and he made a deal for Dean's life he also bargained for Dean to no longer have weight issues. In John's mind, this meant his son would be a muscled ideal soldier but what actually happens is that Dean no longer is subject to any negative issues regarding size allowing him to get to impossible sizes.
Sam and Dean retire and without the pressure Dean starts to grow and he likes it and Sam loves seeing his brother truly happy. Dean gets bigger and bigger and more turned on and eventually they figure it out. Have fun. It could even mean he stays at least partially mobile longer then most people would.
Sam has always been really lanky and when he goes away to Stanford he ends up putting on the freshman 15 (and then some) really quickly. Dean visits from time to time and teases Sam that at that rate he'll end up doubling his weight by the time he graduates. Sam shrugs him off but before finishing his second year at university he's shocked to see he hit that number. Where does he go from there?

Anything where 16 year old Sam is hand feeding his very fat older brother. Could have ust or lead to their first time.

The real reason Dean didn't graduate high school is because he got too fat for school.
