I was thinking Callie as the doctor in the Lynn side since she's a crier. Could be Arizona. Don't think it's Meredith as she usually doesn't cry in front of strangers. Maybe they are doing a trial separation or one of them accepted some something that has them away from home for a specific time frame. :(
Stephanie or Jo would fit with the Dylan / Jack sides -- could see either one of them doing the stare down. Not to say Alex wouldn't be good, just thinking the Residents need something to do and that sounds "fun" for one of their characters.
The Greg, Parker, Mia -- could be just about anyone depending on what's up with Mom. Neuro consult with Amelia or Derek, general with pick your doc.
Always wrong but giving that Lynn side the side-eye. Not liking it one bit if it means Callie or Arizona need help with Sofia ...
Don't like the Lynn side, either. It does sound like Callie. Or how about another Ellis flashback when she moved Mer and her to Boston? Arizona would never leave Sofia. Period. Even if Callie and her separated for good.
Separated: One of them needs help with Sofia and doesn't want to call on the other to help. E.G. it's Callie's turn with Sofia, but work happens and she needs help (a nanny) to cover the unexpected and be available 24x7. Same would apply to Arizona.
Out of Town Work: Alternately, Greys may elect to show the struggle of parenting when parents are in two different locations. With one only available on week-ends or some specific commute schedule. True to life for many. Sometimes it means coming home to spouse and kids once or twice a month.
Together but Overwhelmed: both Callie and Arizona could be sinking in work and juggling schedules, feel they need relief - help via nanny. The wording could intentionally be misleading by implying one vs two.
I doubt either Callie or Arizona would leave Sofia. The only way that would happen is if Sara or Jessica were written off the show.

So this nanny service works for another doctor at the hospital as well? Am I understanding that correctly? maybe this is something both Meredith and Callie (as the leads) are confronting. Another parallel.
Maybe. Or maybe Callie used it back in season 9 when Arizona was in the hospital and she was still a full-time surgeon operating on Derek's hand and trying to save her wife's leg.
We've seen Callie and Arizona discuss their plethora of babysitters before. Remember? Callie couldn't remember their names, "Unibrow" etc. so to have one of them upset about needing a nanny now would be stupid.
Plus it seems like Callie & Arizona live in a very family-friendly neighborhood where they'd have potentially several sitters at their disposal. Whereas Meredith & Derek live in the woods with nobody around them for miles (that's the reason Mer took Zola to her old house for trick-or-treating).
oh idk why but i have the feeling the nanny side is for arizona. Depending on what happens in 11x5 i think it will be clearer if its c/a or someone else. Meredith has had amelia helping with the kids so unless something changed wouldn't be her. The kids i think will be alex and jo is the doctor staring the kid down
leaning towards az but my other guess is maybe april, she barely been in the first few episodes and said april/jackson stuff coming up. idk how far along they are making her in terms of pregnancy but i could see her getting all worked up way early even before the kid arrives.
