I agree with every flail and "guh" and lack of ability to be coherent. That is how I spent most of the episode. I think that the Ackles/Padalecki team really went all out and were fantastic beyond even how wonderful they normally are. Especially Jared. He has kicked his acting up a notch every year and always managed to impress me. But this was full-out-in-league-with-Jensen episode for Jared. This was the first time that I think they really went toe-to-toe on the same acting level. Talk about knowcking it out of the park

Oh and I agree that Sam must be possessed--no detox I know of kicks the shit out of you like that;) I spent most of the episode having chest pains and trying not to cry everywhere--damn Jensen's eyes anyway:P Jared....I am still trying to piece together some sense here. I found a good short meta for any possible meanings of that annoying mirror-cam shot.
I kind of thought later it was a surrogate assault on the viewer's perception of the situation, as attacks on the eye often are in film (like the eyeball being sliced open in Le Chien Andalou), but really, if that's the case it was serious and rather cartoony overkill. We're assaulted already, thanks :-)

I'm pretty sure that was real Dean. Sam's DT hallucinations would have gone away after he got dosed by Ruby.
