And in Dean's defense (just a little bit because I also agree that Madison was Sam, and Sam was Dean doppelganger we had this ep), but Sam did only know Madison a day. He didn't love her as much as Dean loves Sam.
Also, I recently started a Sam/OFC fic about two weeks back, and I named her MADISON. Now I'm going to have to change that. Those writers need to stop stealing our stuff. =P
YES. Wall slam was absolutely hotness, mostly because, yeah, I've written Sam slamming Dean against the wall and tongue-fucking his mouth, and NOW I KNOW EXACTLY WHAT IT LOOKS LIKE. XD

Well, obviously, because no one in the history of the world has ever loved another person as much as Dean loves Sam. :D In all seriousness, though, I wasn't trying to make a parallel in that sense - in fact, I don't think I'd even qualify what Sam and Madison had as "love." The point that I was trying to make (and I think that Sam may have been trying to make) is that this isn't just another monster ...this is a person he's been intimate with, he's shared deep feelings with, and, moreover, who he thought he could save. But he couldn't save her, and when it came down to it, he did what needed to be done.
