I was named after Karel Capek, a Czech playwright & sci-fi author from the 1920s/30s. My parents didn't really fail on most of these (though #2 certainly annoys me to this day) but they did overlook one teeny tiny consideration that I'd like to add to your list.

See, most people who haven't met me will read my given name and make a particular assumption. Therefore I've gone my entire life being called "Carol" by... everybody. (This is usually followed by a look of surprise mixed with chagrin when I turn out not to be a girl.) There have been maybe a dozen people in my entire life who either got my name right the first time or chose to ask before making the attempt ( ... )

I'm named after relatives on both sides of the family, and my middle name is actually olde schoole in its spelling: Malinda. Think, 1890s and before.

Its venerable history does not keep people from calling me Melinda, Belinda, Melissa, Michelle, Monique, Mona, Melanie, Melody, or any of a hundred incorrect permutations.

My favourite was "Maunaa". Say what?

This is why I answer to Milla online. It is at least spelled right, though I suspect that people pronounce it in their heads as MILL-ah and not MEE-lah, as my friends who nicknamed me intended.

In college, we had Lizbuff, Lisbet, Niffer, Jenny, Jen, JenJen, Jennsy, Liz, Lissy, Libby, Beth S., Beth T., Liza, and probably five or six more I fail to recall.

Lots of Jennifers and Elizabeths.

My fave roomie was a Jenny. :)

Went through a drive-through yesterday for our cheeseburger fix. The guy's name was "Phap". We were pissing ourselves laughing as we drove off.

Blech- I had to go through the alphabet and think of names of people that were complete assholes to me in elementary school, otherwise I'd have that association as well. ;)

My new niece is Johanna Christine.

Alas, my most evil co-worker ever (or second most evil) was Johanna.

My first most evil had mental issues, and was a Muriel.

So I will never be allowed to forget the woman who dumped all her work on me, broke into my computer to print out my personal e-mail (sent after hours, during lunch and on breaks) because i wouldn't do her work for her, in an attempt to get me fired. Note that I did both our jobs for several months before she was hired, without shuffling any of it off on anyone else. Note also she would have had plenty of time to do her damn job if she wasn't always out of the office shopping, golfing, lunching, getting haircuts, manicures and pedicures or boinking her abusive boyfriend. Ugh. What a horrible cow.
