The woman in the afterlife resembles Madame Kovarian of Silence/eye drive fame. But isn't. It's who I thought she was.

I love Twelve. I was at Magic City ComicCon this weekend and Colin Baker basically said Twelve is the doctor he wanted to play. As he put it, "Every six doctors, they get it right!" Much more of a throwback to classic Who - a bit like the first Doctor, rather grumpy and not fully "getting" humans. Totally agree with you about Listen - I found the ending especially annoying.

No hints on the woman in heaven - you'll just have to watch and see! :-D
