If you know that they are in fact a Dr., identify them that way. If you are unsure, first name last name or Professor.

As for the picture thing, it just makes me thing of the video application in Legally Blonde. I didn't include a picture with any of my apps.

Don't attach a photo. When we were reviewing resumes for research assistants for our next project earlier this year the resumes with photographs attached got chuckled about, and we did not consider those applicants as seriously (and by we I really mean my professors - I didn't really see what was so funny, but whatevs) - and by consider I mean "didn't take as deep a look at the actual content because they were too busy laughing at the picture." We really don't need to know what you look like when you apply; it's not relevant to your interests or ours, or the quality of your work, and it's not U.S. custom. Many of the applicants who included their pics were from other places.
