Yeah, I find that sometimes when writing deliberate ambiguity, working out what specifically happened is counterproductive, and it's better to just make sure you understand what all of the POV characters believe about what happened.
I'm going to be very pleased about the clinic progress on your behalf.

See that hadn't occurred to me because I was still approaching the damn thing as a straight-up thriller which is STUPID because that's not what I fucking write.

Yeah, "straight-up" is not a label I'd apply to you writing any genre of fiction.
I predict actual helpfulness, and then a frustrating wait that feels unreasonably long, and then more actual helpfulness, and so on, and so on, until medical transitioning is complete.
