Cauldron of Battle Question

Ok, this is a quick question or two about the Cauldron of Battle. Our guild had the 50% increase in flasks made from the cauldrons and 50% increase in buff time from flasks. We are a 25 man raid and drop two of these puppies at a time. We make sure everyone knows that they are out by ready check, and yet people still aren't getting their flasks. Well, to make that clear, only a few people, sometimes 2, sometimes 3 or 4 aren't getting a flask. I know you can double dip from them by using your flask and taking another, do you think that is what is happening to us? With our guild perks we should be getting 15 flasks per cauldron, right? Are there any bugs related to these cauldrons? I just want to cover my bases before jumping to conclusions.
