I'm looking for a specific fic from back in the early years of SPN. It's Sam/Dean where Dean gets injured and is unconscious, and Sam has to patch him up. During this, due to the placement of the wound, Sam discovers that Dean has a really small dick, think micropenis. Sam remembers how John had stopped letting them share baths when Sam got old enough to be curious, to "protect" Dean, I think. In one scene, Sam sees Dean having sex with a woman in an alley, and realises he's using a strap-on. It becomes Wincest, with Sam very into Dean's size.
