I'm not sure whether it was posted on fanfiction.net or AO3.org. It was written between 2009-2014 because I remembered reading it on my IPHONE 4. It never finished, but it was updated once a year or so. When I went to check on it again (2023), it wouldn't show up no matter the key terms I used, so I'm assuming it has either been deleted or has drowned within the depths of the newly created fanfictions.
Details:
In this fanfiction, Sam and Castiel are married, and Dean is their son. Sam and Castiel are in their late 30s or early 40s. while Dean is a teenager or so. They have another son, whose name I don't recall (probably Adam); however, Dean said something to the effect of, "He's like a drunk little man," as a description of his baby brother. Dean is also a boxer who was placed in the sport to help him deal with his out-of-control emotions. A scene I remember was after a fight with Sam, his mother, he punched the bathroom mirror, causing his hand to bleed, and Sam ended up taking him to the emergency room, where they later rekindled.

Summary:
Sam is an unorthodox omega who is stubborn and headstrong. He is married to Castiel, who is accommodating to his demands. They met back in college, and Castiel was amazed by his outspokenness. Castiel gives Sam respect and treats him well, and Sam has become an advocate/speaker for omega rights.
Within the storyline, Dean had presented as an alpha and had begun to have budding feelings for his "mom," Sam. He begins to be more attentive and loving to his "mom," Sam, which makes Sam happy because he felt heartbroken when Dean was in his avoidance phase and felt he was too cool to hang out with his mom.
Dean then begins to change his forms of affection and ways of giving attention, such as by begging to be breastfed like his baby brother Adam or by begging to be kissed. At first, Sam is perplexed by the attention, especially since Dean is no longer a child. However, Sam begins to enjoy the feeling of his son becoming "closer" to him and convinces himself that it's "normal," even if it isn't, as Sam later realizes.
Later in the relationship Dean is constantly testing Sam’s comfort zone. Gauging Sam’s reactions to see whether it’s positive or negative to see if he can keep doing what he’s doing or if he should back off and let Sam get used to the treatment.
There's a scene where Dean buys lingerie for Sam, which Sam is uncomfortable with because he’s always been a masculine omega (about 6ft in height), and Dean has bought him a white sundress, which he isn’t used to, an argument ensues where after a few days of silent treatment Sam eventually relents and wears it for Dean. There's also a scene where Dean sees Sam naked after he finishes showering and eventually has you-know-what with Sam.
All the while, Sam comes to the realization that Dean makes him feel things he's never felt before while being in a relationship with Castiel, and he’s uncomfortable with it. He begins having immense guilt for cheating on his husband and occasionally pushes Dean away by ignoring him to cope with his feelings and actions.
In the midst of this, Dean has a friends-with-benefits relationship with an older girl at his school who is known for being a slut and tells her about his unrequited love, who is a married omega man. Sam later discovers about this relationship during dinner when Dean's friend informs him. Castiel is proud of Dean while Sam’s hurt and confused, but he's not sure why because they aren't even dating, and he chalks it up to Dean being secretive with his relationship instead; they probably had make-up you-know-what afterwards (Dean's way of apologizing).
Later, Sam eventually accepts their relationship and believes that he and Castiel were never mates, and that he had given birth to his mate. That he and Dean were true-mates. The last chapter I recall reading was about Dean being sent abroad to military school or boarding school by Castiel. Castiel coerced Sam to agree because he believes Dean is a problem that needs to be solved. His parents were actually the ones to suggest to him that he send Dean away for his own good. There are constant fights, and Castiel thinks Dean is challenging his authority; however, he's never disciplined him properly to show who's the "alpha" in the household because he believed Sam would freak out about it if he ever found out. When Dean discovers their plan, he is upset and leaves in a fit.

There are a lot of smut scenes in this fanfiction. Sam eventually begins to enjoy being treated like a traditional omega.
