Next week...a trip to Libby, MT

I scheduled an appointment for next week (Mom's is sometime this fall). I received a 19 page questionnaire of info to fill out to take to my appointment. WOW! 19 pages.
CARD (Center for Asbestos Related Disease) states in their letter they are collecting information on everything (sleep pattern/stress/tobacco/depression/occupational exposure/did you fish in certain streams/did you burn firewood/did you interact with mine workers).
