Like if the shoot had to be delayed because there's a problem with the lights, the actor is not supposed to be holding on to the gun or just have it sitting around on a table or something until the issue gets resolved.

Those incidents had blanks in the gun. With Brandon Lee it seems that a small bit of shrapnel from a previous blank was lodged in the gun and then functioned as a projectile that hit Brandon Lee the next time the gun with fired. With Hexum there were blanks but he didn't know the damage the blanks could still do so when he fired the gun at his head the force of the blank did enough damage to kill him because it was point blank.
This incident seems to have ACTUAL LIVE ROUNDS which is a total wtf. How did that happen?
