I made this in PS7, but it is easily transferable to other programs.
All that was used were a few simple layers.
We're starting with this texture by abernathi
Desaturate the texture and duplicate it once, setting the duplicate to screen @ 100%.
Pick out the image you would like to use, prep as necessary, and desaturate it.

Now, past your image onto the texture. Set it to darken @ 100%
Next, I created a new layer, took a fuzzy brush (with the color EDEDED) and drug it across the bottom of my image.
Now, take this texture (I'm not sure who it's by) and past it on your image.
I erased the yellow spot and set it to screen @ 100%
I then created some tiny text and applied it to my icon (font = Sylfean)
