LORs and Book Reveiw

I'm sure this has been asked, but a scan of tagged entries didn't show it:

For some of my schools that require waiver forms, my professors have already handed me signed and sealed envelopes, sans form. One of these is abroad and there's no way on earth I can get a form out of him. Can I write out a "I waive my right..." note, or even put in a filled out form with my envelopes in my packet? Will that be okay? MFA programs at UMass Boston, U New Hampshire and Columbia, fwiw.

Also, Columbia requires "A written response to a book in the applicants chosen genre, written in the past ten years." I am applying for the fiction program, and my SoP is very clear about how I plan to write fiction dealing with disability. One of my creative writing samples deals with this, one doesn't. My choices for this paper are:

A. A novel dealing with a disabled main character. I feel that writing about it might pigeonhole me into "all I can do is disability." Still, this would give me a chance to do "how I would do it differently".

B. A relatively unknown novel dealing with the Armenian genocide. Not my field, but I find it really unique and interesting, and could give some good critique on structure/taking ideas from life into fiction.

Thoughts?
