I think I have, at least for now, found a workable solution to instilling a meaningfully powerful fear of mortal dangers into a kid with no concept of death (AND with an unfortunate fascination for medicine and going to the see the doctor! so "go to the hospital and get shots" is the wrongest choice for a disincentive):
"If you do ____, you can hurt yourself very much, and your legs may not work again. And you won't ever be able to run, or jump, or play, or go to the park again."
(Listing everything that "____" has been filled in with over the past few months would take many pages...)

it's funny, these days everything is friends-only, but somehow I'd forgotten to lock that entry. (let me know if you want me to add you, but I have to warn you most entries are pretty boring.)
