Is it just me, or are the falling people a Junji Ito reference?

Yeah, although with no way to confirm it, I could see that. I don't doubt that SHAFT includes fans of Junji Ito what with the unconventional and horror manga-esque style they dip into sometimes.

Yeah, I think it's quite likely. Especially given how the Kuusou Rumba OP was pretty much a paean to the great guro horrorshow, Litchi Hikari Club.
