I'd say it's pretty much entirely a supernatural infection. IT'S A REALLY WEIRD SITUATION and this actually causes problems when Edgar actually goes to a mental hospital at one point since he's super loopy and they try to diagnose/treat him but nothing can actually stop or help what's currently happening to him since it's all weird supernatural brain parasite stuff.
