As for the pronouns, they are used with other characters as well. I've seen Bakura being called Bakura-san. Paradox is often called as Para-san/-sama or Paradox-san/-sama.

Only when I saw a fanart, let's say, with Seto and Jou, than I saw Jonouchi being adressed with -kun, while Seto wasn't. xD
It might be Yuugi's/Atemu's influence, though, since they call him like that all the time. xD
I just realized it so heavenly because my sister used to like Jou not as much and only when she rewatched some episodes a few years later, she totally started to respect him and called him Jonouchi-kun. xD
