In the Tactics fandom, there's Yoko (Tactics) and Youko (Tactics) - they're the same character.

I'm not familiar with the fandom but Erekos - A. M. Tuomala seems to be there twice with different sets of characters

Achane, Jeiger Staag, Kemaeros, Milaus
Erlen Eppeim, Gamela, Loukaros, Shabane

We had a couple folks in our tagmods who are also AO3 tag wranglers, so they could move the fandoms into the appropriate genres. \o/

Mouretsu Pirates | Bodacious Space Pirates appears to have some duplicates (Chiaki Kurihara = Kurihara Chiaki; Kato Marika = Katou Marika).

The Cal Leandros series has the following characters listed:

Caliban Leandros
Ishiah (Cal Leandros)
Niko Leandros
Robin Goodfellow
Robin Goodfellow (Cal Leandros)

The last two should be combined into Robin Goodfellow (Cal Leandros).
