It's has to be Pestilence, unless Zach was listening in on Dean and Cas' phone conversation, there's no way he would know where Dean is. Though, if it is Pestilence, they're picking them off kind of quickly.
And even if it isn't and Zach does have something to do with Dean's little trip into the future, how is that going to get him to vessel up? Unless Zach thinks seeing how bad Sam and Cas have it, will change his mind. Of course, when has Zach's plans ever really made sense or been really thought through? I think Raphael was right about Zach. He's the angel of failed fan fic plans.
Ok, now remember I've only seen that little 20 sec preview, so I don't know more than that, BUT... Who's to say it was Zach? You said Dean gets whammied after denying Sammy, so what's keeping it from being He who zapped them onto the plane and brought Cas back? I mean, He wants Dean to stop this, and knows Dean will fight to the death for Sam and Cas. He'd also know that Sam and Dean are at their strongest together and together they stand a chance at beating Luci. I think it's a kind of 'you need each other' thing.
And I'm focusing this mostly on the Sam and Dean aspect because that's what I think (atm, at any rate) is meant to be the focus of the ep... even if I, for one, would be more than ecstatic to have it revolve around gun-toting buddhists and their orgies. :P

Later, Dean awakens five years in the future in an abandoned city and is attacked by humans who have been infected with a demonic virus. Zachariah appears to Dean and explains that this is the world that exists as a result of Dean saying no to helping the angels fight Lucifer. Dean meets up with Future Dean, who tells him that the virus is the Devil's endgame for destroying mankind."
Of course, Zach talks to Dean after he gets whammied. But everyone is saying they think it's Zach.
