Sarah knows what is going to happen, but Derek has lived through it and there is a difference. To her, it's a possible reality; to him, it IS reality. It's also why she often stops short in her actions, rationalizing that a person can't be punished for an act they have not yet committed. Derek's rational is it's going to happen and they have to be stopped while they are still thinking about how they will act. So, yes, he's capable of murder, but his actions are for the greater good rather than a result of sociopathic tendencies or selfish motivations.
