The 'when have you received good customer service' question is possibly to test how much you believe in good customer service (i.e. if the concept is important to you you are likely to be able to think of an example), and would give them an idea of what you consider to be good service (i.e. the level at which you may perform).
Unfortunately, my attitude towards customer service is that a person should just try hard at their job and do it properly, rather than attempt to make the experience memorable (in fact, if the experience is memorable then it's probably a sign of bad customer service)
