especially if it's interfaith, you should include any religious customs you plan on including in the ceremony with an explanation so everyone in the congregation knows what's going on.

I found a template online (not interfaith though) and then emailed it to my minister and he tweaked it to how they usually do it. I would start there, and then ask your officiant for the ceremony stuff, and then use the rest of the template to fill in whatever personal info you want.

So I did my (mostly agnostic but interfaith) programs, as well as both of my cousins who also had interface marriages. My cousins had more traditional weddings, so I'll use theirs as a model ( ... )
