A few thoughts:

Cut all stimulants asap.
Exercise 30 minutes in the morning.
Take melatonin before bed to induce metabolic change. Same time every day mind you.

Do nothing in bed but sleeping and sex. No reading no phone no tv.

Even if you can not sleep, establish an evening routine where you take the melatoNin, prepare for bed, lie down and meditate in order to get to sleep for at least an hour. A lot of insomniacs stop trying to sleep which exacerbates the issue.

You need to excercise more. Burn off that excess energy, work out frustrations. Swim, bike, do some cardio. I know you knees aren't great but there are tons of stuff you can do.
