Same as now. :-) Take a look at Queen Victoria - she had a 50 inch waist at one point in her life. She was 81 when she passed away.

The idea that people died young pre-20th century is a bit of a myth. The average age lifespan takes into account everyone that died in childhood and everyone that lived to a normal old age (like 81!). So if you have half the population dying at 5 and the other half dying at 80, you get about an average lifespan of 42.5. That doesn't mean you need to start planning your own funeral at 42. :-)
