Actually the painting isn’t from a scene of the 6x20, but from the 4x16 (the reference pic was a screencap, that’s why I know for sure). Cas is alone, while Dean is torturing Alastair in the next room, just before Anna showed up.
Anyway, I love both episodes, because their hallmark, Cas’s inner conflict, is one of the aspects that for me defines him as a character. That need to do the right thing, even if he makes poor choices more often than not :)
