First off, I started by using this picture from Arwen-Undomiel.com:



Ok! To start with, I made a black backround and then made a new layer where I added my picture. Crop it the way you like. I made sure it was cut on the top and bottom so that the black was showing. =) Hide the backround layer (the black).
As usual, I sharpened my base to 40 and then duplicated it twice.
For the first duplicate, put it to Screen with the opacity at 40.
For the last duplicate, I set it to Soft Light, at 100.
Merge the visible layers. I did this Color Balance:
~COLOR BALANCE~
--Midtones--
Cyan/Red: -100
Magenta/Green: -32
Yellow/Blue: -20


Now, I put the Hue+Saturation up to 60. (That will depend on your picture, fiddle around with that until your satisfied)
Now, go back to your backround layer and show it.
Make a new layer at the top. What I did, was I a fuzzy border around the picture's edges on the top and bottom so as to make them look blended.
Take this texture by sanami276 and set it to Multiply, 100.



Duplicate it and set it to Burn, 100.
Add some scratchy textures and/or words and whala! That tis it! ^_^ For the words, "Heroes" that I used, was Carpenter ICG. :)

For icons that you want to be black and white, duplicate the base and desaturate the duplicate. Cut out the colors on their clothing or whatever you like. Merge and just up the Brightness+Contrast. Follow all the steps (except for the Color Balances and such).
