I do love Margaret Van Eyck's portrait for that style - she looks like she has some serious horns going on, but if you look at the top of her head, you see it's relatively flat.
It's not related, except that Karen also links to it, but I think the way the woman in red is getting the horned effect in Campin's Marriage from a squashed version of the ring-shaped hat is cool, too:
