With makeup effects that look (to my eyes at least) like female genital amputation mutilation gone even more Horribly Wrong™ than FGM already is (assuming that's even possible under the laws of physics as currently constitutes), and some camera trickery for the scene (implied in at least one of their gallery photos) where mega-peen girl j-j-jams it in.

Pretty much. Still, *shrug* somebody finds the 2D version sexy (at least where unfeasibly large ponos is concerned), so I assume, as Iguchi and Nishimura must have assumed*, that some of those people will find it sexy in live-action as well.

* Or they wouldn't have perpetrated this thing.
