For category, do you want the same icon cropped smaller and smaller, so that the crop is the focus, or do you want different icons different sized?

They icons should each have a different "subject" - so different icons in different sizes.

"Fake background" simply means, that you have to exchange the background around the people in your icon preferably with a stock image.

"OTP" means "one true pairing". Most people use it to describe the pairing of a show they "ship", but since not everyone is into shipping, you can also use your favourite pairing or your two favourite characters on the show.
