I get why Tony can't be fully treated in Chaostan, but why not once he gets back? If it takes a week for the shrapnel to work it's way into the heart, and an electromagnet can keep it from doing that; it's operable in the USA!

I thought the implication was the size and number and placement of the shards meant it wasn't operable no matter the technology; best that could be done was to freeze them in place from causing further harm.

Because it wasn't operable way back in 1963 when the character was first created and it never occurred to the film-makers that time has marched on.
