Because of how hormones can influence the Gallbladder, and how fat tissue can effect hormone levels, females as a whole are more at risk, especially if they are overweight, and also especially if they've had multiple pregnancies. Age also plays a role, hence "Forty" (implying forty or older) and for women, once they're much older than 40 hormones become a factor of their own. Flatulence in this case is not a causitive factor but a sign that there is something amiss in the digestive tract. Obviously, flatulence on its own is not enough to go on, since all people produce gas. "Fair" refers a belief that people with pale skin are more at risk, though there seems to be some dispute in the scientific validity of this.
