Letter question

I'm writing a letter (to be signed by my therapist) to bring to the DMV requesting the "M" be changed to "F" on my license. I have my name-change hearing next week, but my last appointment before then is this afternoon.

I know the rules vary by state - but those of you that have used similar letters, what did yours say?

What state do you live in? In CA you have to have a doctor fill out DL 328. Medical doctor (perhaps specifically a physician).

i don't know when i will get my input on this letter thing, i'd have to ask my shrink about it. but i can more than likely find out what the form/information is for illinois, if anyone is interested.
