can you write a side note somewhere explaining that you cannot contact them because of special circumstances but you're more than willing to explain on the phone or in person... and then just write the duties you had?

It's an online application, so there's no way I can add anything or explain this stuff at all...which is part of why I'm so stressed. I feel like they're not really getting any information. The things I put for company name won't really be company names so it's not like they'd know what that meant...my job titles will be pretty vague too...and that's ALL the information they ask for.
